CHDP Dental Training:  Oral Health Assessment and Referral

This training is designed for local CHDP Program staff to use when training CHDP providers and provider staff, or as a CHDP provider self training module.  The goal of the training is explained in the Introduction section below.  A PowerPoint (PPT) presentation and training materials are provided to facilitate and standardize CHDP dental assessment, documentation and referrals.  This training is intended to include all of the following materials, but may be customized to meet the training needs of local CHDP staff and providers.​
