​How Do I Get Medi-Cal as a Former Foster Youth?

1. Did you leave foster care in California at age 18 or older after January 1, 2014?

  • You will be automatically put in the Medi-Cal program for Former Foster Youth (FFY) when you leave foster care and will stay covered until age 26. You do NOT need to reapply. If you have questions or problems with your Medi-Cal, talk to your social worker, probation officer, or Medi-Cal eligibility worker.

2. Did you leave foster care in California at age 18 or older before 2014 or leave foster care in another state or tribe?

  • You can sign up by filling out a 1-page form, called the MC 250A, or by giving your information over the phone to your local county.
  • Apply by phone: Most counties have a specific phone number, staff, or office to help FFY sign up. To find the FFY contact for your county, please download the County FFY Contact List (Not DHCS)​.
