​California Community Transitions Roundtable Meeting Minutes

On the first Tuesday of every other month, the California Community Transitions (CCT) Project Director hosts a Roundtable conference call meeting to facilitate two-way communication between state staff and local service providers, CCT Lead Organizations (LOs).   

The purpose of the Roundtable meetings is to share new CCT Project information and to provide a forum in which CCT LOs can provide feedback to the state.

Minutes are recorded at each meeting and are posted to the DHCS website once they are approved by attendees at the subsequent meeting.
