- Complete physical examination
- Oral health assessment
- Nutrition assessment

- Immunizations as appropriate for age
- Vision screening

- Laboratory tests for anemia, blood lead, tuberculosis, urine abnormalities, sexually transmitted diseases, and other problems as needed

CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services
