Periodic Child Health Assessments/Examinations

  • Health and developmental history
  • Complete physical examination
  • Oral health assessment
  • Nutrition assessment
  • Behavioral assessment
  • Immunizations as appropriate for age
  • Vision screening
  • Hearing screening
  • Laboratory tests for anemia, blood lead, tuberculosis, urine abnormalities, sexually transmitted diseases, and other problems as needed
  • Health education and anticipatory guidance

At the time of the health assessment, CHDP providers make referrals to:

Care Coordination

CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services

School Entry Requirements

California law requires all children entering the first grade to have either a CHDP certificate of health examination or a waiver on file at the school in which they enroll.  CHDP can assist families to meet this requirement by linking families with CHDP providers.
