You must be an authorized party to receive the Medi-Cal beneficiary's information through the Online Inquiry Form links. Authorized parties include the Medi-Cal beneficiary or beneficiary's attorney, a first or third party insurance company, or personal representative for the beneficiary.
Upon receiving your inquiry, DHCS will send a secure email response within 24 hours. We can address these common inquiries through the following Online Inquiry Forms:
- Complete the Medi-Cal Beneficiary Client Index Number Request form if you need the beneficiary's Medi-Cal identification number to submit a notification of claim (new case).
- Complete the Third Party Liability Case Status Request form if you would like to request the status of an existing case.
- Complete the WDP Inquiry form to verify payment for the 250% Working Disabled Program (250% WDP), request a premium payment waiver due to COVID-19, or need assistance with Electronic Funds Transfer payments.
