​​Medicare Medi-Cal Plans

A Medicare Medi-Cal Plan (MMP or Medi-Medi Plan) is a type of Medicare Advantage plan. It is for people who have both Medicare and Medi-Cal. It combines Medicare and Medi-Cal benefits and Medicare prescription drug benefits into one plan.​​ In 2023, Medi-Medi Plans are available in Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo, and Santa Clara Counties. Medi-Medi Plans will coordinate all benefits and services across both programs, including all Medicare and Medi-Cal covered services.

Medi-Medi Plan​s also include:

  • One care team to coordinate care.
  • One health plan to coordinate delivery of services, including medical supplies, transportation, and long-term services and supports.
  • One set of benefits and a network of providers, including doctors, hospitals, clinics, labs, pharmacies, and medical equipment suppliers.
  • ​You may get extra benefits like dental, hearing, or vision coverage, in addition to what Medi-Cal covers.

Important Updates

Beginning January 1, 2024, DHCS will expand the availability of Medi-Medi Plans to five additional counties:​ Fresno, Kings, Madera, Sacramento, and Tulare.​​

Joining a Medicare Medi-Cal Plan​

​On January 1, 2023, Cal MediConnect members were automatically transitioned to Medi-Medi Plans provided by the same plans as Cal MediConnect.

If a beneficiary was not in Cal MediConnect and would like to join a Medi-Medi Plan, there are three options to enroll:

  • ​If already enrolled in a Medi-Cal plan, call the plan directly and ask about their Medi-Medi Plan option
  • Call Medicare at 1-800-MEDICARE
  • Call Health Care Options at 1-844-580-7272 (TTY 1-800-430-7077).​

​Beneficiaries can join a Medi-Medi Plan ​​if they:

  • ​Have both Medicare and Medi-Cal
  • Are 21 years or older
  • Live in one of these counties: Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo, or Santa Clara
  • Dual eligible beneficiaries who reside in the new expansion counties can choose to enroll in a Medi-Medi Plan during the 2023 Medicare Annual Enrollment Period beginning October 15, 2023.​

PA​CE

​Program of All-Inclusive Care for the Elderly (PACE):  PACE covers Medicare and Medi-Cal benefits, including prescription drugs.  It will coordinate healthcare, homecare, transportation, and dental care. To find out if PACE is available in your area, and to enroll, please go to the CalPACE website. ​

SCAN Fully-Integrated Dual Eligible Special Needs Plans (FIDE SNPs)

​SCAN Connections and SCAN Connections at Home are Medicare Advantage Special Needs Plans that covers Medicare and Medi-Cal benefits, including prescription drugs. The plans will coordinate all healthcare, homecare, transportation, and related benefits. To find out if SCAN Connections at Home is available in your area, and to enroll, please go to the Scan Health Plan website​.

To learn more about Medi-Medi Plans, stakeholders can participate in the bi-monthly CalAIM Managed Long-Term Services and Supports and Duals Integration Workgroup, which is open to the public. The workgroup is a collaboration hub where stakeholders can offer feedback to the Department of Health Care Services (DHCS) and engage in discussions related to the policy, operations, and strategy for the D-SNP transition.

Stakeholders can also participate in the quarterly All-County Duals Communications Workgroup. The workgroup offers a neutral platform for stakeholders to discuss Medi-Medi Plans and the statewide D-SNP transition. They are currently offered to stakeholders who serve dual eligible beneficiaries in the counties that offer Medi-Medi Plans and the 2024 expansion counties.
