I have questions about the PASRR Determination
General Questions
1. What is the PASRR Determination?
The PASRR Determination contains placement and treatment recommendations for the individual based on the Level II evaluation. DHCS clinical staff create this individualized letter that becomes available online to the facility.
2. How long does it take to receive a PASRR Determination?
Federal mandate requires the PASRR Determination be available within an annual average of seven to nine working days from the date the Level I Screening was submitted.
What to do with the PASRR Determination
1. How does the conservator get a copy of the PASRR Determination?
The facility can print and provide a copy of the PASRR Determination to the appropriate parties listed on the distribution list of the letter, including any other person involved in the care and treatment of that individual.
2. I am at a Medicaid nursing facility. What does my facility do with the Determination?
You must file the determination in the individual’s medical record and provide a copy to the: 1) individual 2) individual’s legal representative 3) admitting nursing facility 4) individual’s attending physician; and any other person involved in the care and treatment of the individual 5) send it with the Field Office with the TAR.
Concerns regarding the DHCS PASRR Determination
1. I do not agree with the recommendations on the Determination. How can I appeal?

2. My facility will not be providing some of the services recommended in the PASRR Determination. What do I do?
If your facility does not provide a recommended service, which was included in the PASRR Determination, please document the reasons in the individual’s treatment notes.
