This webinar was to review the draft concept paper for ACA Section 2703 Health Homes for Patients with Complex Needs (Health Homes Program) hosted by the California Department of Health Care Services (DHCS) in coordination with the California State Innovation Model (CalSIM) initiative. During the webinar, we shared the Health Homes program concept in order to obtain stakeholder feedback.
The Health Homes Program is an optional Medicaid benefit that will coordinate access to medical and behavioral health services and long-term services and support, and provide linkages to community social supports. The Health Homes Program was a key care improvement initiative in the CalSIM Testing Grant application to the Center for Medicare and Medicaid Innovation (CMMI).
